Jack Gerome Formy-Duval, age 84 of Chadbourn, slipped into eternity on Tuesday morning, November 9, 2021, at Duke Raleigh Hospital.
Jack was born on September 26, 1937, in Columbus County, to the late James Cecil Formy-Duval and Carrie Emma Gore Formy-Duval. He was also preceded in death by his siblings, James “JC” Cecil Formy-Duval and Monte Formy-Duval.
Graveside services will be held at 11:00 a.m. on Saturday, November 13th, 2021, at Piney Forest Baptist Church Cemetery.
Jack retired from the United States Army, serving as a Lt. Colonel for 29 years.
Survivors include his wife of 60 years, Priscilla Faye Carter Formy-Duval of the home; daughter, Kelly F. Jones (Russell) of Wilmington; brother, Gene Formy-Duval of Southport; grandson, Sawyer Duval Jones.
